Find Your 408(b)(2) Fee Disclosure

The 408(b)(2) disclosure spells out who is getting paid  in your 401k plan. The first step toward understanding your fees is to find your 408(b)(2), and while providers are required to furnish these disclosures, they don’t always make it easy to locate. Select your record-keeper below and follow the simple directions to find yours or simply call the number provided. Then upload your fee disclosure below to have our staff benchmark your costs and identify unnecessary fees.

  1. Log in to Fidelity

  2. Click on the ‘Plan information’ tab

  3. You’ll see a long list of options, one of which should be the ‘fees in the plan’.

  1. Log in to ADP

  2. Hover over the ‘Resources’ on the left side

  3. Click ‘Plan Information’, then ‘Fee Disclosure’

  1. Log in to Voya

  2. From the orange menu bar at the top of the screen, select ‘Plan info’

  3. Click ‘Sponsor Disclosure’, followed by ‘Participant/ Plan Sponsor Disclosures’

  1. Log in to John Hancock

  2. Hover over ‘Your Resources’

  3. Click on ‘updates to 408(b)(2) disclosures’

  4. Your fee disclosure will open in a new window

  1. Log in to Ascensus with a User ID and Master PIN

  2. Click ‘Plan Disclosure’

  3. Click on the latest Participant Disclosure in the box at the top right.

  1. Log in to  Rowe Price

  2. Hover over ‘Accounts’

  3. Click ‘Statements / Documents’

  4. Then select ‘Plan Forms’

  1. Log in to Ubiquity

  2. On the menu on left side, click ‘Overview’

  3. You should see your plan’s fee disclosure

  2. Click on ‘My Accounts’, then ‘Statement’ from the dropdown

  3. Click on ‘Required Disclosure’

  4. Click on ‘Participant Fee Disclosure’

  5. Scroll to the bottom and click ‘administrator fee disclosure’.

  4. First item under plan documents should be fee disclosure
